Rash on my hands

About two weeks ago I started to have a red blotchy rash on one hand in the same places where it is swollen.  On top of the rash there are small blisters.  The rash is itchy and has the same feel as a mosquito bite.  Now I have it on both hands.  The blotchy areas are quite symmetrical.

Is this a scleroderma symptom?  Is this the beginning of hardening of the skin?  What cream or medicine should I use for the itchy feeling?

By the way, I am on 400 mg plaquenil, 40 mg Protonix. I just got off my 5 mg prednisone. About three weeks ago, I started Diflucan 200 mg once a week and Progresterone 50 mg daily. The blotchy rash covers half of my hands.

I worried if this would affect my upcoming AP treatment. I hope it won't screw me up.
