This just came in on the CTLyme yahoogroup and may be helpful to those using Doryx, which is apparently easier on the stomach than doxycycline due to its delayed-release form. Doryx can be costly and this is a pdf form for download to apply for a $50 rebate (valid through to Dec '08).
